We are participating in the Student Health Survey, sponsored by the Oregon Health Authority and Oregon Department of Education.  The survey will be conducted online on Friday, November 7th for all 6th and 8th grade students, with a makeup day soon after.  The survey is completely anonymous and students do not have to answer any questions they are uncomfortable with.  The survey asks questions about student health and safety, mental and behavioral health, school climate and culture and risk and protective factors.  You may opt your child out of the survey by sending the attached opt out form to the FRMS office anytime before the day of the survey, and your child will have the chance to refuse participation at the time of the survey.  You only need to submit the form if you are opting your child out.

The results of the survey give us valuable information that help us understand the needs of our students as a whole and help guide our decision making for continual improvement in instruction and school climate.  In addition to the opt out form, you will find attached a copy of the 6th and 8th grade surveys for you to review if you would like.  If you have any questions, feel free to contact either Mrs. Johnson or Mr. Carman.
